Follow the steps below to complete your Supplemental Packet:

STEP 1:

Use and return the following checklist below to ensure that you have completed, acknowledged, reviewed, and have enclosed the required documents / forms listed below:

STEP 2:

Read the following:

• FDLE Policy 3.4 - Standards of Member Conduct

STEP 3:

Complete the following forms:

• FDLE OEI 43 - Non-Sworn & Contractor Applicant Background Waiver
• FDLE OEI 37 – Authority for Release of Information for Current/Future Spouse, Domestic Partner, Roommate(s) and/or Family Members(s) Who Reside With You For 6 Months or More
• FDLE OEI 36 – Disclosure Pursuant to the Fair Credit Reporting Act (FCRA)
• Photograph (Approximately 2" X 2")
• FDLE OEI 32 – Non-Sworn Supplemental Application (DO NOT print documents two-sided.)
• Copy of Birth Certificate
• Copy of Valid Driver License
• Copy of Social Security Card
• Documents for Highest Degree Earned (i.e., copy of high school diploma or an officially, sealed college transcript). If transcripts are ordered, request that they be sent to the address given below.
• FDLE OHR 063 – Military Service Information Form (Required by all applicants.)
• Affidavit-of-Non-Service (Required by all non-military applicants.)
• Copy of Selective Service Registration Card or Proof of Exemption From This Requirement (All males born on or after October 1, 1962.
• Copy of DD214 with Discharge (Previous or current military only.)
• SF 180 - Request Pertaining to Military Records (Required by previous or current military only.) Fill in the information for numbers 1-6 of Section I and number 1 of Section III, sign the form, send the form certified mail via the appropriate address listed on page 3 of the form, and provide Capitol Police with a copy of the form with the date and tracking number of your certified mail.
• FDLE OEI 38 - Acknowledgement of Receipt and Review of FDLE Drug Free Workplace/Drug Screening
• OHR 104 – Notice of Social Security Number (SSN) Collection
• DOJ 361 - Certificate of Identity (Required by applicants who have been born, lived, or worked in a country other than the United States of America.)
• Florida Retirement System (FRS) Certification Form
• Copy of Name Change Documents (i.e., court order, divorce decree, marriage certificate, etc.)
• CP120 – Tattoo Acknowledgement Receipt
• FDLE Security and Privacy Training for Members
• FDLE OEI 44 Fingerprinting Waiver

STEP 4:

Submit completed packet of all required documentation to the address listed below:
